This position is available for remote work on a full-time basis within Arizona (including virtual office arrangements).

The State of Arizona strives for a work culture that affords employees flexibility, autonomy, and trust. All work, including remote work, should be performed within Arizona unless an exception is properly authorized in advance.

Job Duties:

Accepts admission calls/inquiries. Meets with referral sources, family members/responsible parties and applicants interested in nursing home placement. Conducts facility tours. Provides prospective residents with pre-admission packets. Completes Admission Screening Checklist. Determines need for nursing home level of care and ability to cover cost.

Upon receipt of application and supporting documents or forms, reviews information for completeness. Works with applicants to complete/obtain information required for admission to include DD214, proof of freedom from tuberculosis, proof of payer and other clinical and financial information deemed necessary by the Admissions Committee.

Compiles, organizes and presents applications to the Admissions Committee for approval.

Maintains data on census (inquiries, admissions, discharges, referral sources). Maintains required records. Prepares and distributes reports. Attends meetings and completes required in-service training. Participates in special projects as assigned by the Administrator.

Visits and otherwise communicates with current and potential referral sources to provide education regarding benefits available to veterans and to market ASVH services. Presents ADVS/ASVH information to potential customers and to a variety of audiences. Reports inquiries, community contacts, hospital visits and number of tours given, along with disposition of each to Administrator on a weekly basis.

Distributes completed admission paperwork to various departments to ensure timely billing, placement of documentation in medical chart and continuity of care.
Actively involved in AMS processes such as huddle boards, problem solving, Job Breakdown Sheets, success tracking, and standardized work. Participates fully in the QAPI process, our Quality Assurance and Performance Improvement program mandate by CMS, which incorporates AMS.

Knowledge of:
State administrative rules and policies
Medical terminology
Medical conditions associated with residents of long-term care/ skilled nursing facility
Federal and state benefits available to veterans
Reimbursement systems used by the facility in obtaining payment for care provided
Community resources that may be used as potential referral sources
The veteran community

Skills in:
Verbal and written communication skills
Active listening skills
Assessing medical and behavioral conditions to determine eligibility for services
Understanding and describing various reimbursement systems to consumers
Gathering, organizing and analyzing information
Public speaking/presentation skills
Evaluating, developing, and implementing marketing strategies
Preparing a variety of marketing and informational materials

Ability to:
Speak and write clearly, concisely, and effectively
Deal with varied emotional states of potential residents/family members
Provide supportive information to families that might be in crisis
Establish and maintain effective working relationships

Selective Preference(s):

Experience in admissions within in a long-term care/skilled nursing facility

Pre-Employment Requirements:

Ability to obtain and retain a valid AZ DPS Fingerprint Clearance Card
Ability to drive in the course of State business. Requires possession of and ability to retain a current, valid state-issued driver's license.
Current Negative Tb skin test (Mantoux skin test) within the last 6 months or a written statement from a physician, Physician’s Assistant, or a Registered Nurse practitioner indicating freedom from tuberculosis, if in the past has had a positive skin test or tuberculosis
