Job Summary:

This is an advance job posting for the Arizona State Veteran Home – Flagstaff. First review of applications will be conducted after 1 November 2022.

The ideal candidate will assist the Director of Nursing Services in operating the Nursing Department of the Arizona state Veteran Home - Flagstaff, a skilled nursing facility. Assumes responsibility for clinical outcomes of the facility and oversees nursing practices of the Registered Nurses, Licensed Practical Nurses, and Certified Nursing Assistants. Manages supervisory staff responsible for scheduling and assigning duties to nursing personnel. Establishes and manages facility-wide nursing care indicators, ensuring that the facility complies with federal and state guidelines governing the outcome of Veteran/Member care. Incorporates a cultural focus on Veteran/Member-centered care in the performance of job duties. Actively participates in the Arizona Management System (AMS). Learns, implements and monitors AMS methods. Utilizes AMS concepts and tools for problem solving and continuous work/process improvements and efficiencies. Occasionally drives on State business; completes special projects; and performs other job-related duties.

Job Duties:
  • Supervises and manages the use of scheduled staff throughout the facility to ensure proper use of resources, adequate staff coverage, and competency of assigned staff.
  • Evaluates the quality and outcomes of critical nursing functions as they pertain to Veteran/Member care.
  • Takes necessary actions to train and supervise staff to ensure that all standards of care are met on a daily basis.
  • Applies working knowledge of all applicable regulatory requirements to evaluate comprehensive functions of the Nursing Department and institutes corrective action whenever deficiencies are identified.
  • Participates and assumes leadership role as assigned on committees and task forces which may include Quality Assurance, Infection Control, Pharmacy, Incidents/Accidents, Admissions, and Safety.
  • Participates in the development policies and procedures.
  • Ensures compliance with federal, state, and VA regulations and facility policies and procedures.
  • Participates in Human Resources activities related to interviewing, selection, performance evaluations, coaching/counseling and administering disciplinary action as necessary.
  • Responds to and takes appropriate action regarding emergent issues that involve allegations of abuse, neglect/mistreatment of residents, fire or police involvement, and elopements; and ensures the Director of Nursing Services and/or Administrator is immediately informed of developing situations.
  • Provides assistance on units or with supervision as required; and may act on behalf of the Director of Nursing Services in their absence.
  • Actively involved in AMS processes such as Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized work.
  • Participated full in the QAPI process, the Quality Assurance and Performance Improvement Program mandate by CMS, which incorporates AMS.
Knowledge, Skills & Abilities (KSAs):

Knowledge of:

  • Advanced nursing knowledge related to clinical management of moderate to complex medical conditions found within a skilled nursing facility
  • Knowledge of employee management issues related to the specific demands of a nursing home environment, including knowledge of scheduling, training, supervision, and application of nursing practices
  • Knowledge of all relevant regulations as they pertain to the nursing home, including Omnibus Budget Reconciliation Act (OBRA), Occupational Safety and Health Administration (OSHA) and other standards
  • Knowledge of regulatory requirements and how to complete resident assessments. Minimum Data Sets and the design and implementation of resident care plans
  • Knowledge of human resources functions including recruiting, interviewing, training, coaching/counseling and taking disciplinary action
  • Knowledge of communication approaches/ techniques, including the effective use of English in oral and written form, commutation styles effective in meeting or conference settings and approaches useful in communicating with culturally diverse populations
  • Knowledge of design and implementation of systems related to workflow, scheduling, quality assurance functions and tracking the frequency and prevalence of quality indicators within the nursing home Environment

Skills in:

  • Skill in managing systems, functions, material/supply resources, human resources and nursing practice
  • Clinical skills necessary for assessment and planning appropriate medical interventions, especially as they pertain to quality indicators such as pressure sores, nutrition, dehydration, incontinence, therapies, end of life issues and appropriate use of medications
  • Fundamental nursing skills that may be used on an intermittent basis when providing nursing care or supervising and assessing the work and practice of other nursing staff
  • Skill in problem solving in areas of complex or multi-faceted issues as they relate to nursing care and practice, internal/external resources and important family/resident issues
  • Management/leadership skills
  • Skill in establishing and maintaining interpersonal relationships at all organizational levels

Ability to:

  • Ability to learn, interpret, and apply State Personnel Rules
  • Ability to learn and apply agency policies and procedures
  • Ability to act professionally and compassionately with both internal and external contacts
Pre-Employment Requirements:
  • Must possess an active license to practice as a Registered Nurse in the State of Arizona
  • Must be able to obtain and retain an AZDPS Fingerprint Clearance Card
  • This position is required to drive in the course of State business, and requires possession of a current, valid state-issued driver's license
  • Current Negative Tb skin test (Mantoux skin test) within the last 6 months or a written statement from a physician, physician’s assistant or a registered nurse practitioner indicating freedom from tuberculosis, if in the past has had a positive skin test or tuberculosis
