The Arizona Pioneers' Home is a continuing care retirement home operated and funded by the State of Arizona. The Home provides care and services to residents living independently, as well as to those requiring personal, assisted living, intermediate and skilled care.

Job Summary:

The Caregiver shall be responsible for the provision of such assistance as is needed to the residents of Arizona Pioneers' Home Including, but not limited to, assisting with feeding, hydration, bathing, dressing, incontinence care, oral care, personal hygiene, infection control, positioning, transferring, ambulation, maintenance and cleanliness of personal areas, measuring of vitals to include oxygen saturation, pulse, blood pressure, respirations, height and weight under the direction and supervision of the unit nurse and in accordance with the plan of care.

The Caregiver shall be responsible for documenting such in accordance with APH Policy and Procedure. The Caregiver shall be observant and knowledgeable in resident care and be able to identify significant changes of condition, vital changes or abnormalities and safety concerns and report such to the unit nurse for follow-up and direction. The Caregiver shall perform such duties within the scope of practice of a Certified Nursing Assistant under the Arizona State Board of Nursing rules and in accordance with all applicable laws, rules and regulations while following the individual resident's Plan of Care.

The Caregiver shall be familiar with and perform his/her duties within the policies and procedures of APH with special regard to issues of abuse, fall risk, skin breakdown issues, infection control, blood and bodily fluids precautions, employee, resident and visitor safety.

The Caregiver shall endeavor to provide an inviting, friendly and homelike environment to the residents of the Arizona Pioneers' Home while ensuring the highest quality of life and independence for the residents of Arizona Pioneers' Home.

Job Duties:

• Provision of assistance in the activities of daily living to all residents to include positioning of bed bound residents, hygiene and incontinence care while providing privacy with adherence to infection control policies and In accordance with the plan of care. These tasks shall be performed with a high degree of attention to the safety of the resident as well as the Caregiver

• To ensure quality and continuity of care the Caregiver shall perform rounds and/or report at the beginning and end of each shift with off going and oncoming Caregivers

• The Caregiver shall monitor Arizona Pioneers' Home residents for changes in condition and safety risk's to residents, employees, visitors and vendors and communicate concerns to the nurse in charge

• The Caregiver shall ensure cleanliness of residents, resident living areas and work areas by removing trash, sending linens and personal clothing to laundry and cleaning as needed in accordance with APH policy and procedure. The Caregiver will also ensure their work area is well stocked in anticipation of resident needs

• The Caregiver shall assist in the collection of lab specimens as ordered and within the scope of practice and under the direction of the supervising nurse, taking of vitals and monitoring of urinary input-output as ordered and documentation of such in adherence with Arizona Pioneers' Home policy and procedure

• The Caregiver shall be responsible for and familiar with the Arizona State Board of Nursing scope of practice, Arizona Pioneers' Home policy and procedure and a minimum of two hours per month in service training

• The Caregiver shall present a professional demeanor and interact with residents, co-workers, residents' families, visitors, volunteers and vendors while maintaining confidentiality of medical information

• Attending to the basic needs and desires of the Arizona Pioneers' Home residents and providing or communicating those needs and desires in accordance with policy and procedure

Knowledge, Skills & Abilities (KSAs):

Knowledge:

• Braces, splints, prostheses, slings, walkers, wheelchairs, lifts, anti embolism stockings, assistive devices such as hearing aides, oxygen delivery systems and other medical equipment

• Knowledge of provision of activities of daily living with the ability to individualize the care according to the plan of care

• Measuring of vitals and ability to identify and communicate those not within normal limits or a significant change

• Transfer and positioning techniques with high regard to safety of the employee and resident

• Medical issues allowing the Nursing Assistant to be able to identify changes in condition

• Nutritional and fluid requirements and knowledge of feeding procedures

• Basic infection control procedures and universal precautions

• Professional oral and written communication skills

• Basic documentation standards

• Resident hygiene procedures

• Incontinence care and risks

• Specimen collection

Skills:

• Application of prostheses, anti-embolism stockings and oxygen delivery systems and other ordered medical devices

• Skin care in relation to both incontinence and bed-bound residents

• Written, oral and visual communications

• Indwelling urinary catheter care

• Appropriate bed making

• Safe transfer techniques

• Measuring of vitals

Ability:

• Ability to safely provide assistance in the activities of daily living including but not limited to dressing, bathing, toileting, oral and personal care and feeding

• Interact with all residents, employees, family members, visitors, volunteers and vendors in a professional manner

• Demonstrate the ability to communicate in writing and verbally in the English language

• Achieve a higher level of knowledge and skills through in-service and other training

• Ability to perform essential functions in a safe, efficient manner

• Provide flexibility in work hours and days

• Maintain strict resident confidentiality

• Follow written and verbal directions

• Be punctual with good attendance

• Work in a team environment

Selective Preference(s):

• Current certification as a Nursing Assistant (CNA) under the Arizona State Board of Nursing or compact state

• High School diploma or equivalent. Additional post high school education especially in the nursing or medical field especially preferred

• Flexibility in the work schedule to include varying shifts, work days and hours as needed

• Prior experience as a Caregiver, CNA, PCT or related experience

• Excellent interpersonal and communication skills

Pre-Employment Requirements:

• Provide fingerprint clearance by Arizona Department of Public Safety within twenty days of hire

• Provide documentation of freedom from active pulmonary tuberculosis before hire

• Must not have been convicted of a felony or conviction involving moral turpitude

• Must be 16 years of age or older
