The Department of Economic Security, Division of Developmental Disabilities (DDD) is seeking an experienced and highly motivated individual to join our team as a Case Manager 3 – Early Intervention Unit. This position serves persons with developmental disabilities (DD) and Mental Illness (MI) by supporting them and their families to lead the healthiest, most-sufficient and productive lives within their capabilities.
This position serves children with developmental disabilities and their families. The purpose of early intervention is to support the family in their relationship with their child with a delay or disability. The end result of early intervention is to develop a firm foundation, that is convenient and attainable for the family, upon which the family may challenge and nurture development to build a child's independence and well being in all environments throughout their lifetime.

Accountable for the
• Eligibility determination for the Division of Developmental Disabilities (DDD) and AZ Early Intervention Program (AzEIP)
• Driving vehicle to conduct home visits
• Development of the Individualized Family Service Plan (IFSP)
• Coordination of identified supports and services for families and children of greater complexity
• Transition out of early intervention

Acts as the Lead Worker in the Unit and is responsible for more complex and high-profile cases. As Lead Worker, serves as back-up to the Supervisor.

Job Duties:
  • In compliance with requirements (including mandated timelines), drive vehicle to conduct home visits and meet with individuals/families to develop relationships and become familiar with family dynamics/priorities/resources/concerns/supports and make appropriate referrals to community agencies.
  • Convene and facilitate multi-disciplinary Individualized Family Service Plan (IFSP) meetings (a minimum of every 6 months on every child) to support children with developmental disabilities and their families to work toward successful acquisition of outcomes within the child’s natural environment.
  • Offer the family (at the IFSP) the AzEIP surveys; educate parents of their rights and responsibilities and procedural safeguards. Teach parent advocacy skills. Assist families with the grieving process when applicable. Collaborate directly with Child Protective Services (CPS) in meeting a child’s needs while in the CPS system. Assess each child’s progress on a regular basis.
  • Research community for available resources to meet needs identified in Plan. Inform family of resource options. Assist family in choosing appropriate providers. Assist family with enrollment into the DDD Early Intervention Program and secure identified/needed services for all children (DDD & ALTCS).
  • Confer with supervisor; attend staff meetings, training, and other meetings as required. As lead Support Coordinator, will direct employees and represent the unit at all meetings, trainings, etc. in the absence of the supervisor.
  • Gather all necessary information and documentation for determining eligibility for DDD Early Intervention services, screen with the Ages & Stages Questionnaire. Use assessment tools. Ongoing assessment of developmental levels. Complete all necessary intake processes in line with AzEIP & DDD timelines, including AZ Long-Term Care System (ALTCS) application on all intakes.
  • Take the lead in scheduling preschool transition conferences with the child’s local school district for transition into the public school system. Attend Individualized Educational Plan meetings with the local school. Assist the family in transitioning from the AzEIP system to the public school system.
  • Write referrals for chosen providers to ensure that funding is secured for services. Assist the family with orientation into all systems, any problem solving, & system issues which may arise.
  • Determine DDD eligibility for children birth to three years of age. Provide “interim service coordination” for children and families who may not be eligible for DDD. Complete redeterminations when appropriate.
  • By law, take part in required child find activities. Educate community regarding purpose of early intervention services and structure of early intervention system including AzEIP and its participating agencies (AZ School for the Deaf and Blind, AZ Health Care Cost Containment System, AZ Department of Education, & DDD).
  • Assist the family in enrolling in any of the AzEIP participating agencies for which they may be eligible. Network within collaborative community work groups and communities. Develop and maintain files for each child. Write comprehensive progress notes and maintain case records. Prepare a variety of reports and complete special projects.
Knowledge, Skills & Abilities (KSAs):
  • Knowledge of methods, procedures, and techniques used in the development of Individual Support Plans (ISP)
  • Knowledge of Federal and State statues and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities
  • Knowledge of principles and practices of case management and community resources
  • Skilled in oral and written communication
  • Skilled in developing, evaluating, and maintaining member service plans
  • Skilled in interviewing, counseling, and interacting with members and family members
  • Ability to work independently and within diverse work teams
  • Ability to document case files and information within required/mandated timelines
  • Ability to maintain confidentiality
Selective Preference(s):
  • One year experience working with people with Developmental Disabilities preferred.
Pre-Employment Requirements:
  • A minimum of a Bachelor’s degree in Early Childhood, Early Child Special Education, Family Studies, OR a Bachelor’s degree in a closely related field (e.g., Psychology, Social Work, Elementary Education, or Sociology) with review of course and approval by ADES/AzEIP; OR if hired in the AzEIP system as a Service Coordinator before July 1 2001, documentation of continued satisfactory work in the AzEIP system in a Service Coordinator position from that date of hire.
  • In-state travel is required. This position requires possession of and ability to retain a current, valid state-issued driver’s license appropriate to the assignment. Employees who drive on state business are subject to driver’s license record checks, must maintain acceptable driving records and must complete any required driver training (see Arizona Administrative Code R2-10-207.12.). Employees may be required to use their own transportation as well as maintaining valid motor vehicle insurance and current Arizona vehicle registration; however, mileage will be reimbursed.
  • Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults.
  • Candidates for this position shall be subject to a search the Child Protective Services Central Registry pursuant to A.R.S. 8-804.
  • Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired state employees
  • Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency’s ability to reasonably accommodate any restrictions

Critical Services Premium Pay Stipend

This position may be eligible to receive a temporary stipend of 10% in addition to the base pay beginning in December 2021. If eligible, the temporary stipend amount will be calculated using the employee's base rate of pay and will be reflected in each paycheck. It is anticipated that this temporary stipend will be available through June 24, 2022.
