Job Summary:

The Resource Coordination Lead provides guidance to Resource Liaisons who offer initial care coordination to caregivers of children and youth in out-of-home care statewide upon enrollment to the Arizona Department of Child Safety Comprehensive Health Plan (DCS CHP). The position supports team efforts to connect directly with caregivers of children and youth newly placed in out-of-home care to welcome them to the health plan, explain their benefits, set expectations for Early and Periodic Screening, Diagnostic and Treatment (EPSDT) visits for well child, dental and mental/behavioral health, and answer their questions. Additionally, the Resource Coordination Lead provides guidance to team members when working with medical/behavioral health professionals, caregivers (kinship, foster parents, congregate care staff, etc.), clinic/hospital/specialty care providers, and community resources, including for children with special health care needs.

Job Duties:
  • Lead efforts with daily operations, assist other team members as needed with any issues/concerns reported by caregivers and/or field staff with direct supervisor and coordinate incoming work with direct supervisor including but not limited assigning staff and validating team data collection/tracking tool(s).
  • Reviews work products and analyzes information obtained from caregivers with direct supervisor and provides input on situations reported.
  • Produces, records, tracks and reports data related to care management/coordination including referrals made to community based resources including family/peer to peer support agencies/organization using the Arizona Management System.
  • Writes reports summarizing activities or productions covering a specified period of time or marking the completion of a projects.
  • Analyze technical training needs and execute training plans with direct supervisor (including but not limited to coaching/training new staff, review/research learning opportunities to benefit the team and promote process adherence).
  • Other duties as assigned as related to the position.
Knowledge, Skills & Abilities (KSAs):

Knowledge of:

  • AHCCCS contractor/subcontractor standards, practices and compliance obligations
  • Pediatric medical terminology
  • Integrated pediatric health care services coordination
  • Clinical standards of care and quality of care
  • Medicaid and Early Period Screening and Diagnostic Treatment (EPSDT) federal requirements

Skills in:

  • Prioritizing multiple responsibilities to effectively manage time and meet deadlines
  • Computer software such as Microsoft Word, Excel, Outlook and Teams

Pre-Employment Requirements:

The ability to secure and maintain clearance from the DCS Central Registry.
