Job Summary:

This Clinical Care Specialist performs statewide care coordination activities to service providers, community partners, and guardians of children and youth in out of home care throughout their involvement with DCS. This position supports the delivery of integrated (physical health and behavioral health) health care and resolution of concerns pertaining to access to health care services and system navigation within the Department of Child Safety Comprehensive Health Plan (DCS CHP) in accordance with the Arizona Twelve Principles of the children's system of care.

Job Duties:
  • Responds to internal and external stakeholder requests for support or care coordination for child-specific or system related needs to promote timely and effective operation of DCS CHP and Mercy Care DCS CHP within standards of care. Develops professional partnerships with internal and external colleagues. Organizing and managing a high-volume email inbox while responding timely, in accordance with process and policy.
  • Provides technical assistance, support, and advocacy as part of a member of the CHP System of Care team to support care coordination and care management activities for the delivery of physical and behavioral health care for children involved with DCS.
  • Manages CHP System of Care projects, coordinating action plans, amongst DCS CHP team members.
  • Maintains up-to-date knowledge of Arizona Cost Care Containment System (AHCCCS), DCS and DCS CHP policies and procedures and Current and emerging laws, regulations, and professional standards at the federal, state, and local levels, and the ability to apply relevant guidelines and best practices to ensure compliance and high-quality service delivery.
  • Other duties as assigned as related to the position.
Knowledge, Skills & Abilities (KSAs):

Knowledge of:

  • Applicable laws and regulations
  • Government (Medicare, Medicaid, Supplemental Security Income (SSI), and Social Security Disability Insurance (SSDI) and insurance benefits including AHCCCS Medical Policy and Contractors Operations Manuals
  • Current and emerging laws, regulations, and professional standards at the federal, state, and local levels, and the ability to apply relevant guidelines and best practices to ensure compliance and high-quality service delivery

Skills in:

  • Assessing, determining, and evaluating appropriate disposition and making independent judgments based on critical thinking skills and expertise
  • Demonstrating professional behavior, promoting cooperation and team building
  • Communicating effectively and professionally using all modalities i.e. technology, written, and verbal with internal and external stakeholders in a way that is both clear and concise

  • Work independently with minimal direction
  • Anticipate and organize work flow
  • Work with a high volume request for assistance or information
  • Deal effectively with rapidly changing priorities and follow through on responsibilities
Selective Preference(s):

The selected candidate must possess a Bachelor’s degree in social work, psychology, sociology, or a related field, as well as 1–2 years of experience using Microsoft Office.

Pre-Employment Requirements:

Employee must obtain and maintain clearance of the DCS Central Registry. Employee must obtain and maintain an Arizona Fingerprint Clearance Card.
