This position supports the Housing and Health Opportunities (H2O) Program Manager by coordinating projects, monitoring deliverables, and ensuring program activities are implemented as designed. Serving as a key liaison among internal leaders, the Program Administrator, and external partners, the role promotes alignment, communication, and collaboration. The Project Coordinator tracks progress, identifies gaps or risks, supports data monitoring and reporting, and helps maintain program fidelity and continuous quality improvement. This role requires strong organization, problem-solving, and relationship-building skills, along with a commitment to improving outcomes for AHCCCS members.

Major duties and responsibilities include, but are not limited to:
• Project Coordination: Plan, organize, and manage projects that require collaboration across various units, divisions, agencies, both internal and external, and provides assistance and support to the H2O Program Manager. Lead and manage updates to H2O documents and templates, ensuring they are current and effective.
• Data Management: Track program progress, analyze data and trends, identify gaps, escalates concerns and risks, and provides detailed reports to the H2O Program Manager. Identify and escalate trends or concerns that may require policy or process improvements to ensure intended program outcomes are achieved.
• Oversight and Monitoring: Perform site visits, monitoring, and oversight to ensure quality in service delivery and compliance. Maintain and work with AHCCCS internal program leaders to ensure compliance and performance of the required duties of the H2O Program Administrator and the H2O Providers. Organize and maintain files and records related to H2O.
• Liaison: Serve as the primary point of contact between internal Program Leaders and H2O Program Administrator and external system partners, facilitating effective communication and collaboration. Works cross functionally to prepare, distribute and communicate project status, problem solve, and implement approved changes.
• Policy Review: Review and ensure that current practices are accurately reflected in existing policies, recommending updates as necessary.
• Meeting Preparation: Schedule and coordinate on-site visits as needed, prepare meeting agendas for H2O Program Manager, and compile necessary documents and materials for meetings.
• Special Projects: Undertake special projects as assigned by H2O Program Manager and actively participate in relevant meetings.

Qualifications:

Minimum:
• Three or more years of working in housing and homeless services, case management, care coordination, social services, and/or behavioral health services. Understanding of the Arizona Medicaid System and the homeless service continuum.

Preferred:
• Bachelor's Degree in Public Health, Social Work, Psychology, or a related field, and 2+ years experience in housing and homelessness services, case management, care coordination, social services, and/or behavioral health services is a plus.

Pre-Employment Requirements:

• Successfully pass fingerprint background check, prior employment verifications and reference checks; employment is contingent upon completion of the above-mentioned process and the agency’s ability to reasonably accommodate any restrictions.
• Travel may be required for State business. Employees who drive on state business must complete any required driver training (see Arizona Administrative Code R2-10-207.12.)
