The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Habilitation Supervisor 1. DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high quality, and member driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community
• The position assists in the development and implementation of Individualized Support Plans (ISPs); administers medications; participates in discussions and coordinates plans; functions as a member of an interdisciplinary team and ensures that the health, safety, and psychological needs are met for the members with developmental disabilities who live in the home.
• Completes performance evaluations (MAP); approves ETE Time Cards for staff; facilitates shift meetings; communicates with staff regarding job performance; completes work schedules and assures adequate staffing levels at all times; participates in interviews and problem-solving personnel issues. Communicates pertinent information to staff related to their job & individuals for whom they are responsible
• Completes review of Client Incident Reports to ensure all information is accurate, complete with results sent to the supervisor. Ensures calls are made to administrator on member incidents as required by policy during the same shift. Ensures required documentation is completed on a daily basis (e.g., Time Cards, program data, treatment sheets, member fund requests/reconciliations, etc.).
• Demonstrates leadership by displaying a positive attitude, promoting teamwork, ensuring compliance with policies and procedures, problem solving, giving recognition, and striving to improve services. May perform housekeeping duties, meal preparation, and other miscellaneous duties.
• Knowledge of agency rules, regulations, policies and procedures
• Knowledge of Federal Title XIX ICF/IDD regulations Individual Support Plan (ISP) process Members' rights and advocacy
• Experience working with specialized needs for individuals with intellectual disabilities
• Knowledge in principles of behavior management and positive behavioral supports
• Supervisory principles and problem solving
• Skilled in oral and written communication, listening, and problem solving
• Skilled in multi-tasking and prioritizing
• Skilled in coordinating and monitoring the implementation of prescribed therapeutic routines
• Skilled in teamwork and team building
• Ability to evaluate staff performance and resolve problems
• Ability to manage emergency situations in an effective and professional manner
• Ability to monitor the provision of care to individuals, including identifying problems and implementing solutions
• Ability to manage a diverse group of staff to develop goals and teamwork within the shift to accomplish those goals
Candidate must be 18 years of age and able to read and write in English. Must be willing to work weekends, holidays, and overtime, when necessary. Preference given to candidates with a High School or General Education Development (GED) diploma.
• Employment may be contingent upon completion of a post-offer medical/physical examination and the agency's ability to reasonably accommodate any restrictions.
• Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults.
• Candidates for this position shall be subject to a search of both the Child Protective Services Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459.
• In-state travel is required. This position requires possession of and ability to retain a current, valid state-issued driver's license appropriate to the assignment. Employees who drive on state business are subject to driver's license record checks, must maintain acceptable driving records and must complete any required driver training (see Arizona Administrative Code R2-10-207.12.).
• Employees may be required to use their own transportation as well as maintaining valid motor vehicle insurance and current Arizona vehicle registration; however, mileage will be reimbursed.

This position may be eligible to receive a temporary stipend of 10% in addition to base pay beginning in December 2021. If eligible, the temporary stipend amount will be calculated using the employee's base rate of pay and will be reflected in each paycheck.
